This article shows how funding research on Alzheimer's disease became a priority for the British Medical Research Council in the late 1970s and 1980s, thanks to work that isolated new pathological and biochemical markers, and showed that the disease affected a significant proportion of the elderly population. In contrast to histories that focus on the emergence of new and competing theories of disease causation in this period, I argue that concerns over the use of different assessment methods ensured the MRC's (...) immediate priority was standardising the ways in which researchers identified and recorded symptoms of Alzheimer's disease in potential research subjects. I detail how the rationale behind the development of standard assessment guidelines was less about arriving at a firm diagnosis and more about facilitating research by generating data that could be easily compared across the disciplines and sites that constitute modern biomedicine. Drawing on criticism of specific tests in the MRC's guidelines, which some psychiatrists argued were "middle class biased", I also show that debates over standardisation did not simply reflect concerns specific to the fields or areas of research that the MRC sought to govern. Questions about the validity of standard assessment guidelines for Alzheimer's disease embodied broader concerns about education and social class, which ensured that distinguishing normal from pathological in old age remained a contested and historically contingent process. This article details the relationship between history and bioethics. I argue that historians' reluctance to engage with bioethics rests on a misreading of the field as solely reducible to applied ethics, and overlooks previous enthusiasm for historical perspectives. I claim that seeing bioethics as its practitioners see it – as an interdisciplinary meeting ground – should encourage historians to collaborate in greater numbers. I conclude by outlining how bioethics might benefit from new histories of the field, and how historians can (...) lend a fresh perspective to bioethical debates. (shrink)

Nursing and nurses have become reliant on qualitative methods to understand the meaning of nursing care, and many nurse researchers use Heideggerian Interpretivist phenomenology approaches. Often these nurses are unaware of Martin Heidegger's role in the German National Socialist Party of the 1930s and his allegiance to fascist ideology. We ask: can a bad person have good ideas? In line with pragmatic thinkers such as Richard Rorty, we argue that instead of value judgements on people and their ideas, nurses should (...) consider ideas as a product of a historical/social and political time and space. In urging a critical political engagement, we argue for a Husserlian approach. In opposition to Heidegger's interpretivist phenomenology approach, in which the hegemony of the day is integral to the phenomena being studied, we propose that a more expressive, collaborative engagement using Husserlian descriptive phenomenology approach would serve better in encouraging a more critical engagement with how ideas are used by groups of people, how some groups might be advantaged and others disadvantaged. Our conclusion is that the separation of ideas from political and social context is dangerous, and nurses, including nurse researchers, must understand how their research ideas and methods influence and are influenced by political agendas. This book offers the first sustained critique of individualism in psychology, a view that has been the subject of debate between philosophers such as Jerry Fodor and Tyler Burge for many years. The author approaches individualism as an issue in the philosophy of science and by discussing issues such as computationalism and the mind's modularity he opens the subject up for non-philosophers in psychology and computer science. Professor Wilson carefully examines the most influential arguments for individualism and identifies the (...) main metaphysical assumptions underlying them. Since the topic is so central to the philosophy of mind, a discipline generating enormous research and debate at present, the book has implications for a very broad range of philosophical issues including the naturalisation of intentionality, psychophysical supervenience, the nature of mental causation, and the viability of folk psychology. Individualism in psychology is the view that mental states must be individuated so as to be intrinsic to particular individuals. This view has been thought to impose an intuitive and plausible constraint on explanation in psychology. The dissertation is a sustained examination of individualism, especially with respect to its role in psychological explanation. My particular interest is in showing that individualism is not a constraint on psychology which follows from either psychology's scientific nature, or from the nature of causation or (...) materialism. ;Apart from an extended introduction in which I explain why individualism is both a substantive and plausible constraint on taxonomy and explanation in psychology, the dissertation has three parts. The aim of Part One is to reconstruct and critically discuss three influential, recent arguments for individualism: an a priori argument , a empirical argument , and a methodological argument. I argue that each of these arguments is unsound and should be rejected. In an Appendix to Chapter 2 I argue that individualism does not follow from the causal theory of properties. ;In Part Two I turn to examine individualism itself. The aim of this part of the dissertation is to address the view that psychological kinds and explanations which are not individualistic cannot be part of an adequate account of mental causation. Chapter 5 both responds to particular charges that have been made about denials of individualism, and articulates and criticizes a central intuition about the local nature of causation from which individualism has sometimes been thought to follow. Chapter 6 focusses on folk psychology and attempts to show how one can view folk psychological explanation as unproblematically causal even though it is not individualistic. ;In Part Three I develop a core case against individualism using the concepts of causal depth and theoretical appropriateness, concepts which form the basis for two constraints on good causal explanations. I argue that individualism should be rejected because individualistic explanations of behavior, including individualistic psychological explanations, are not always the causally deepest or most theoretically appropriate explanations available. Recent books, articles and plays about the ‘immortal’ HeLa cell line have prompted renewed interest in the history of tissue culture methods that were first employed in 1907 and became common experimental tools during the twentieth century. Many of these sources claim tissue cultures like HeLa had a “troubled past” because medical researchers did not seek informed consent before using tissues in research, contravening a long held desire for self-determination on the part of patients and the public. In this article, (...) I argue these claims are unfair and misleading. No professional guidelines required informed consent for tissue culture during the early and mid twentieth century, and popular sources expressed no concern at the widespread use of human tissues in research. When calls for informed consent did emerge in the 1970s and 1980s, moreover, they reflected specific political changes and often emanated from medical researchers themselves. I conclude by arguing that more balanced histories of tissue culture can make a decisive contribution to public debates today: by refuting a false dichotomy between science and its publics, and showing how ethical concepts such as informed consent arise from a historically specific engagement between professional and social groups. During the late 20th century numerical rating scales became central to the diagnosis of dementia and helped transform attitudes about its causes and prevalence. Concentrating largely on the development and use of the Blessed Dementia Scale, I argue that rating scales served professional ends during the 1960s and 1970s. They helped old age psychiatrists establish jurisdiction over conditions such as dementia and present their field as a vital component of the welfare state, where they argued that ‘reliable modes of diagnosis’ (...) were vital to the allocation of resources. I show how these arguments appealed to politicians, funding bodies and patient groups, who agreed that dementia was a distinct disease and claimed research on its causes and prevention should be designated ‘top priority’. But I also show that worries about the replacement of clinical acumen with technical and depersonalized methods, which could conceivably be applied by anyone, led psychiatrists to stress that rating scales had their limits and could be used only by trained experts. This article briefly outlines Ørsted's early aesthetic thought by placing it in the context of his affiliations with early German romanticism, and by examining the poetics and philosophy of language contained in a prize-winning essay on aesthetics that he wrote in 1796. Further, this article presents an example of how aesthetic and linguistic strategies in his writing helped shape the meaning of the theoretical terms utilized in his early scientific work. Toward this end, the focus of the article will be (...) limited to a central passage from the first installment of his Kemiske Breve (Chemical Letters), which he published in 1798 at the age of twenty-one. Interface theory is the hypothesis that inferential, representational theories of perception entail that fitness, not truth, dictates the evolution of perceptual systems. They show, with simulations, that “veridical” perceptual mappings (ones that preserve at least some of the structure of the world) are routinely out-competed by “non-veridical” interfaces (ones that make no attempt to preserve that structure). They therefore take particular aim at the direct perception, ecological approach to perception and work to show that such a system, even if technically (...) an option, would never be selected for by evolution. This paper defends the ecological approach from this novel, existential attack by showing that the ecological hypothesis is so different in kind to the inferential, representational view of perception that it simply falls outside the scope of interface theory’s critiques; ecological psychology remains a viable scientific endeavor. This analysis will show that, far from being a radical new approach to perception, interface theory is simply a clear and elegant formalization of mainstream representational psychology, and any implications interface theory may have belong solely to that branch of science. Misleading information pervades marketing communications, and is a long-standing issue in business ethics. Regulators place a heavy burden on consumers to detect misleading information, and a number of studies have shown training can improve their ability to do so. However, the possible side effects have largely gone unexamined. We provide evidence for one such side-effect, whereby training consumers to detect a specific tactic (illegitimate endorsers), leaves them more vulnerable to a second tactic included in the same ad (a restrictive qualifying (...) footnote), relative to untrained controls. We update standard notions of persuasion knowledge using a goal systems approach that allows for multiple vigilance goals to explain such side-effects in terms of goal shielding, which is a generally adaptive process by which activation and/or fulfillment of a low-level goal inhibits alternative detection goals. Furthermore, the same goal systems logic is used to develop a more general form of training that activates a higher-level goal (general skepticism). This more general training improved detection of a broader set of tactics without the negative goal shielding side effect. (shrink)

_341_ _Objectives: _In patients with multivessel disease both the detection of the culprit lesion and the exact allocation are important preconditions for sufficient treatment and improved outcome. In a vessel based approach the combination of quantitative coronary angiography and fractional flow reserve measured by a pressure wire should be advantageous compared to myocardial SPECT, as morphological and functional information is delivered simultaneously. Therefore our aim was to evaluate MS in the detection and allocation of hemodynamically significant stenoses obtained by the (...) combined gold standard QCA plus FFR in patients with multivessel disease. _Methods: _FFR and QCA of 95 vessels in 80 patients have been evaluated. Hemodynamically significant target lesions were defined as intermediate stenoses between 50 and 75% with an FFR. (shrink)

Recent data show that catastrophic events during one cell cycle can cause massive genome damage producing viable clones with unstable genomes. This is in contrast with the traditional view that tumorigenesis requires a long‐term process in which mutations gradually accumulate over decades. These sudden events are likely to result in a large increase in genomic diversity within a relatively short time, providing the opportunity for selective advantages to be gained by a subset of cells within a population. This genetic diversity (...) amplification, arising from a single aberrant cell cycle, may drive a population conversion from benign to malignant. However, there is likely a period of relative genome stability during the clonal expansion of tumors – this may provide an opportunity for therapeutic intervention, especially if mechanisms that limit tolerance of aneuploidy are exploited. Historians agree that the second half of the twentieth century saw widespread changes in the structure of biological science in universities. This shift was, and continues to be, characterized by the de-differentiation of nineteenth and early twentieth century disciplines, with increasing emphasis on the methods and authority of molecular fields. Yet we currently lack appreciation of the dynamics that underpinned these changes, and of their tangible effects on the working practices of those involved. In this article we examine the wholesale (...) reform of biological science at the University of Manchester, England, that occurred in two successive steps in 1986 and 1993. We examine how reform was enabled by economic and political factors, as staff seized upon national pressures; in so doing, we emphasize how this reform was shaped by a generational view of the biological sciences as a one field, unified by molecular techniques. We assess how the success of these reforms was tied to new management policies that rewarded research activity in molecular fields, and refigured teaching as a punishment for research inactivity. We close by showing how our analysis fits amongst, and can contribute to, ‘big picture’ debates in the history and sociology of knowledge. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:On PartnershipRyan Schwarz, Duncan Smith-Rohrberg Maru, Dan Schwarz, Bibhav Acharya, Bijay Acharya, Ruma Rajbhandari, Jason Andrews, Gregory Karelas, Ranju Sharma, and Mark ArnoldyRecently, Bayalpata Hospital, in the rural district of Achham, Nepal almost collapsed under the weight of its own staff's discontent. The hospital had been largely abandoned until 2009 when our organization, Nyaya Health, renovated and opened it in partnership with the Nepali government. Since then, the (...) hospital has seen great progress and has experienced widespread community support. Nonetheless, earlier this year, a broad-based staff revolt occurred, led primarily by our senior clinical staff members, raising concerns that Nyaya's management policies were ineffective, and wages and benefits were too low. The unrest included everything from a strike, limiting services to our patients, to aggression against non-strikers and slander and racist comments in public media against the Nyaya Health Board of Directors. Ultimately, the conflict was resolved, though not without the departure of our three most senior clinicians. The incident deeply damaged staff morale [End Page 101] and impeded basic services to our patients, and from our perspective as a leadership and management team, it was deeply troubling and spiritually challenging. We have learned much in the wake of the strike, but here we focus more narrowly on the concept of partnerships in our work by examining the recent strike and the two key issues that lead to it: wages and benefits, and management policies at Bayalpata Hospital.In grappling with the recent staff crisis, some of our leadership team expressed concerns that we would never be able to develop a true partnership in Achham if our senior-level employees led strikes against the organization. The leaders of the strike were our most well-educated and high-ranking staff. They were also individuals who had come to work for us specifically citing the moral imperative of our work, and their pride in helping to serve the poorest of the poor. One leadership team member asked, "What is 'partnership' then? We came into this work with the premise that we would work together, with our partnership built around the right to health—but who are our partners if they lead strikes damaging the very services our collective mission aims to offer?"Another leadership team member commented, "The first time I visited Achham I was invited for tea at Meena's* house (one of Nyaya's midwives). We arrived at her house and Meena greeted us warmly. She showed us in to a small room: there was no furniture, but there was an immaculately swept mat which we joined her on. We drank tea, seated on her rug, and listened to her tell us how she had come to live in the house, how she had come to work for Nyaya, and her pride in now being able to provide both for her own children, as well as, the children of her relatives. As the only employed family member, she now had the challenge of providing for all of them. When we question our partnership, I'm taken back to Meena's warm, but extremely poor, home. She has worked for Nyaya for almost four years. She has helped our organization provide free care to over 75,000 people. Is this not a type of 'partnership'?"Allow us to offer some background on the issue of wages—Meena has been with our team since 2008, and has been paid about $2,500 per year—a generous wage compared to other employers in the region. Meena was one of the striking staff members. Among other concerns about the way the hospital was being run, she and other staff asked indignantly why we couldn't provide them with higher wages, better benefits, and more resources, both for her and the hospital. Notably, Meena's salary (and the rest of our staff's) was well above her equivalent in other, more urban hospitals (to compensate for the extremely remote area in which we work), and in a district where most families are farmers, our staff's salaries occupy the highest income bracket in the region.
